Sign up for the My Life: Going FAR email discussion group.   This group is for sharing questions, ideas, and sucess stories related to self-directing support services and for supporting people to self-direct and use the Maryland New Directions Medicaid Waiver.

Individuals with developmental disabilities, families, and interested advocates or supporters are welcome and encouraged to participate.

Is Your Plan Self-Directed?

You probably already have a service plan developed. You may be very happy with the plan, or you may want to see it changed. This checklist is designed to help you decide if you want to use your current plan, or you want to develop a new plan for this waiver.

If your plan is self -directed it should include:

  • Information about what is really important TO you and your family? The things you like to do, what you like, the people you care about?
  • Information about what is and what is not working in your life
  • Great Things About You, what you are good at, what you are proud of, what you like about yourself
  • An action page that explains clearly how you are going to get the life you want, and who is going to do what to help you get there.
  • People who are willing to work with you as partners in making your plan happen.

If your plan is not self-directed - it may include:

  • Information that Other People think is important FOR you or your family that you do not agree with
  • It is filled with information about all the things you do wrong or cannot do
