Resource Coordination A Vital Partner in New Directions

 

 

Expertise in Helping You Identify Needed Services

A Resource Coordinator will be involved with you from the beginning planning stages. The Resource Coordinator will adjust their level of involvement in response to the amount of help that you want or need.

Planning for Services

Your Resource Coordinator will help coordinate development of a person-centered plan. The plan will be developed by you, your family, friends or anyone else that you want involved and the Resource Coordinator will help you in the way you want. The plan will be dynamic and responsive to the changes all people experience in their lives.

Accessing Funding

Your Resource Coordinator will assist you prioritizing what services you need, and will help in the budget development process. This budget will be based upon your individual plan.

Designing Services

Your Resource Coordinator will play an important role in helping you decide how you would like your overall services to look. They may help you identify a Support Broker, if you have not already found one. They may also be a critical link between you and the Regional Office if you so choose.

Important Quality Role

The Resource Coordinator will monitor the IP with your help to make sure that services provided are helping you to achieve the outcomes identified in the plan. The Resource Coordinator may also check-in periodically to make sure that you are happy with your services, including your Support Broker. The Resource Coordinator can also play an important role in reporting areas of concern to DDA.
