Exploring the Fabric of Person-Centered, Individualized Services
Person-Centered
Planning becomes another hollow promise when it is not embedded in
courageous commitments to organizational change that lead to values
driven, individualized supports. Beth
Mount, one of the original creators of Person Centered Planning will
discuss with Pat Fratangelo, the visionary leader of Onondaga Community
Living, a model program of values driven individualized supports, the
complex challenges, and subsequent conversations that are common to
agencies that aspire to "One Person at a Time" vision and
implementation. These
conversations are remarkably different from the dialogue within
traditional services. Join us
to explore these different conversations, and leave with some fresh ideas
for exploration.

Person at a Time
This
session addresses some of the factors that provide opportunities for
people with severe disabilities to establish their won homes in local
communities without relying on congregation as a means to save dollars.
This session discusses the strategies employed by one agency to
accomplish supports without congregation and within the average costs for
supported living in one state. The
session is primarily directed
to people with disabilities, their families and other supported living
agencies.

Community Integration - Transition from Institutions to Independent
Living
This
workshop details the process individuals of all ages and disabilities take
as they transition from nursing homes and institutions into the community.
It contains short presentations, followed by a time for hands-on
participation and input from the audience.

Community-Centered Strategies for Supporting and Including People with
Disabilities: The Next
Generation of Natural Supports
Supports
for individuals with disabilities, although increasingly community-based,
operate within a formal human service system outside of community
decision-making structures. Research
suggests that this current system has had limited effectiveness in
advancing the social inclusion of individuals with disabilities.
This presentation will draw from current work in the fields of
community development and planning to outline a new approach that actively
engages communities in planning supports and encouraging the full
participation for citizens with disabilities.
This model, field-tested in a small rural community in
Northern New Hampshire
, resulted in increased municipal-level activities targeted at
accessibility, employment, housing, transportation, recreation, and
community supports.

will share how they have personal involvement with their healthcare
services and offer details on: 1) coordination of health information; 2)
best practices for communication with their support team & medical
