Audio-Visual Equipment Order Form It is the responsibility of session chairpersons to order and pay for additional audiovisual equipment needed for individual sessions through TASH. Because of the tight schedule it is possible that equipment ordered by someone else may be in the room during your session, however that is not guaranteed. Please order equipment now to avoid on-site labor penalties and to ensure availability. You may order online below or a copy of the AV order form can be downloaded here, or you can contact Denise Marshall to request a copy. Should you have any questions or changes needed on-site, please go to the TASH information booth, which will be located near the registration area in the Exhibit Hall. All audio visual orders must be pre-paid in U.S. dollars. Please sign the form and return it with payment before November 1, 2005. Due to increased processing costs, orders received after that date will be charged 25% above the listed rate. Cancellation after that date will be refundable up to 50% of the order.
